As this project started as a PhD dissertation, I express my great thanks to_x000D_<br /> my first supervisor, Birgit Krawietz, whose provided criticism and constructive_x000D_<br /> advice on writing a dissertation which is not only enjoyable for expert readers,_x000D_<br /> but also for non-expert readers. Without her suggestions, I would have ignored_x000D_<br /> the importance of an international audience.
